Service Provider
This online Letter of Authorization (LOA) form service is provided by MocTel inc., a telecommunications company incorporated under the laws of the Province of Quebec, Canada.
Purpose of Data Collection
The information collected through this form is used solely for the purpose of generating a Letter of Authorization (LOA) to facilitate the porting (transfer) of telephone numbers between telecommunications service providers. The data collected includes:
- Personal identification information (name, company name, title)
- Contact information (email address, phone number)
- Service and billing addresses
- Current provider account information
- Telephone numbers to be ported
- Electronic signature (if provided)
- Technical information (IP address, browser type, referring website)
Data Storage and Retention
Submitted form data and generated documents are stored on servers located in Canada operated by MocTel inc. Data is retained for the period necessary to process the porting request and for record-keeping purposes in accordance with applicable telecommunications regulations.
Data Sharing
Your information may be shared with:
- Your current telecommunications service provider (as part of the porting process)
- Your new telecommunications service provider
- Wholesale telecommunications providers involved in the porting process
MocTel inc. will not sell, rent, or otherwise disclose your personal information to third parties for marketing purposes.
Applicable Law
This service and the collection of personal information are governed by the laws of the Province of Quebec and the applicable federal laws of Canada, including:
- The Act respecting the protection of personal information in the private sector (Quebec)
- The Personal Information Protection and Electronic Documents Act (PIPEDA) (Canada)
- The Civil Code of Québec
9-1-1 Emergency Service (CRTC notice)
Telephone service provided over voice-over-IP (VoIP) differs from traditional telephone service with respect to 9-1-1:
- Emergency call routing relies on the service address you register for each number. After a number transfer, its 9-1-1 address does not follow the number: you must register it, and keep it up to date whenever you move.
- 9-1-1 service may be unavailable during a power outage, an Internet outage, or if your service is suspended.
- If your registered address is out of date, emergency services may be dispatched to the wrong location.
These limitations follow from the CRTC's requirements applicable to VoIP 9-1-1 service. Register and verify your 9-1-1 addresses in your MocTel client portal.
Limitation of Liability
MocTel inc. provides this LOA form generation tool on an "as is" basis. MocTel inc. makes no warranties, express or implied, regarding:
- The success or outcome of any porting request
- The timelines for completing a number transfer
- Any service interruptions that may occur during the porting process
- Any fees or charges imposed by your current or new service provider
- The accuracy or completeness of the information provided by the user
MocTel inc. shall not be held liable for any direct, indirect, incidental, special, or consequential damages arising from the use of this service, including but not limited to loss of service, loss of business, or any costs associated with the porting process.
The user is solely responsible for the accuracy of the information provided and for understanding the implications of submitting a porting request, including potential early termination fees from their current provider.
Electronic Signature
If you provide an electronic signature through this form, you acknowledge that it constitutes your consent and authorization as described in the LOA document, in accordance with the Act to establish a legal framework for information technology (Quebec).
Your Rights
In accordance with Quebec privacy legislation, you have the right to:
- Access your personal information held by MocTel inc.
- Request correction of inaccurate information
- Request deletion of your personal information, subject to legal retention requirements
